The Aquinas Office of Outreach and Engagement collaborates with many community-based networks on the local, state and regional level that participate in a number of community-based networks across the state, each representing a social impact focus area, such as economic development, children and families, and social equity. Each network listed here has its own structure, governance, and criteria for participation. The Gather Resources & Align Community Efforts (G.R.A.C.E ) “Network” is an alliance of community-based organizations (CBOs) in Kent County working to provide social services, healthcare and educational support services to individuals in the community. Community Rebuilders has initiated the launch of this network to promote collaboration, efficiency, and person-centered care among all participants, and connect families experiencing homelessness to services that address their Social Determinants of Health (SDoH) and ultimately make family homelessness rare, brief and nonrecurring.  Over time, it is expected that the Network will grow, adding new partners and populations to be served.
